Guidelines for Using CAMlabs™ Services
Customers Using Comprehensive CAMlabs™ Services:
  1. In order to receive the appropriate device from an unmodified model, the customer must provide the following by fax or mail:
    1. The Cover Sheet form.
    2. The Price Options form.
    3. The Measurements and Information form, indicating whether CAMlabs's™ or the customer’s modification technique is to be used..
    4. A negative model made to CAMlabs™ specifications.

  2. In order to receive the appropriate device from measurements, the customer must provide the following by fax or mail:
    1. The Cover Sheet form.
    2. The Price Options form.
    3. The Measurements and Information form, indicating whether CAMlabs’s™ or the customer’s modification technique is to be used.

  3. If a negative model is sent, CAMlabs™ will image the model at our central fabrication facility. All models are modified based on the information provided by the customer on the Measurements and Information form.

  4. CAMlabs™ will mill the modified model and perform the appropriate central fabrication procedures as specified by the customer on the Price Options form.

  5. CAMlabs™ guarantees a turn around time of three days from the time we receive the items listed in number one above to the time the finished product is mailed.
Customers with a BioSculptor Workstation but without an Imager:
  1. The customer must provide the following by fax or mail:
    1. The Cover Sheet form.
    2. The Price Options form.
    3. The Imaging form.
    4. A negative model.

  2. CAMlabs™ will image the negative model at our central fabrication facility using the information provided by the customer on the Imaging form and send the patient file back to the customer via modem within 24 hours.

  3. The customer will modify the patient file, create a fabrication file, and send the fabrication file back to CAMlabs™ via modem.
  4. CAMlabs™ will mill the modified model and perform the appropriate central fabrication procedures as specified by the customer on the Price Options form.

  5. CAMlabs™ guarantees a turn around time of 24 hours from receipt of the fabrication file to the time the finished product is mailed.
Customers with a BioSculptor Imager and Workstation:
  1. The customer must provide the following by fax or mail:
    1. The Cover Sheet form.
    2. The Price Options form.

  2. The customer will create a fabrication file, and send the fabrication file to CAMlabs™ via modem.

  3. CAMlabs™ will mill the modified model and perform the appropriate central fabrication procedures as specified by the customer on the Price Options form.

  4. CAMlabs™ guarantees a turn around time of 24 hours from receipt of the fabrication file to the time the finished product is mailed.
All Customers:
  1. Prosthetic foam models will be supplied unless otherwise specified by the customer. Orthotic foam models will be supplied only at the request of the customer. CAMlabs™ will store models for 30 days.

  2. All models will be milled using a standard resolution unless otherwise specified by the customer.
